We’re seeking applicants for the American Heart Association’s Social Impact Fund who are addressing social determinants of health challenges impacting residents in Florida, specifically within these counties: Charlotte, Collier, DeSoto, Glades, Hardee, Hendry, Highlands, Hillsborough, Lee, Manatee, Pinellas and/or Sarasota. We’re looking for sustainable solutions focused on access to health care, housing, and affordable and healthy food; economic empowerment; or closing the educational achievement gap in under-resourced communities. Applicants may be non-profit organizations or for-profit social enterprises.
